Francesca Sio, who was 3 years old, died after a diagnosis of cancer was missed. A coroner initially refused to hold an inquest to investigate the death. However, after an expert report was obtained from Dr Sandie Bohin and a Judicial Review was issued in the High Court, it was agreed that an inquest would be held.
